How do I access this service?
The following people can be referred by their Doctor or Practice Nurse to the Diabetes Clinical Nurse Specialist:
- Adults with Type 1 Diabetes who have poor glycaemic control
- People with Diabetes who experience frequent or severe hypoglycemia
- People with Type 2 Diabetes who have HbA1c>75mmol/mol, have attended a Diabetes self management course and have Diabetes complications requiring review
Your Doctor or Practice Nurse can refer you to Podium Podiatry if needed.Diabetes Retinal Screening
Your Doctor or Practice Nurse can refer you to Tauranga Eye Specialists if needed. The service is delivered at Māori Health Services premises, Whakatāne Hospital.
The Diabetes Nurse holds weekly clinics at Whakatāne Hospital to provide education and to support people self-manage their Diabetes (home visits are not provided with this service). The Diabetes nurse works with other health professionals including General Practice Doctors and Nurses to promote wellness for people with Diabetes.Diabetes and Heart Health Self-Management
